On December 1, ISIS fighters carried out a large attack on the Syrian Arab Army (SAA) and its allies in the southern Deir Ezzor countryside.
According to the Syrian Observatory for Human Rights (SOHR), the attack targeted several positions of the army and its allies in the vicinity of the city of al-Mayadin.
“Clashes between the two sides lasted for about an hour, accompanied by heavy machine guns fire and shelling, after this, the fighters of the organization [ISIS] withdraw to the desert,” the SOHR’s report reads.
Three fighters of Iranian-backed Liwa Fatemiyoun were killed and ten others were injured as a result of the attack. The group is known to be affiliated with the Islamic Revolutionary Guard Corps (IRGC).
Liwa Fatemiyoun has been deeply involved in operations of the SAA against ISIS in the governorates of Homs and Deir Ezzor.
The attack was most likely launched from the Homs desert, which lays between eastern Homs and western Deir Ezzor. Hundreds of ISIS fighters and commanders are reportedly hiding there.

If you mean Idlib, or any other areas controlled by terrorists, the terrorists use civilians as human shields, building their ammo dumps and other infrastructure in civilian neighborhoods. That is why Syria is slow to simply bomb them to smithereens (as the US did in Raqqa and Mosul, killing thousands of civilians).
